Application forms and registers
All applications in respect of approvals/renewals/extensions as a test certifier (including periodic tester), applications for a CSL for explosives and vertebrate toxic agents or applications for ‘Compliance approvals’ for plant and equipment used in workplaces (see list below) must now be made to WorkSafe New Zealand. The compliance approvals include:
- Issuing test certifier approvals, including renewals and extensions
- Oversight of the test certification regime, including responsibility for the test certificate register, the annual audit programme, workshops, investigating complaints etc
- Issuing controlled substances licences (CSLs), including maintenance of the CSL registers
- Issuing approvals for plant and equipment used in workplaces and maintenance of the various registers
- Approving codes of practice relevant to workplaces
- Provision of guidance and information relevant to hazardous substances used in workplaces.

Application forms and registers

- a compliance plan for a stationary container system, a secondary containment system, or a tank wagon
- a variation to a separation distance or other matters
- a waiver for removing a disused underground tank
- a variation to, or a waiver of, fire-fighting provisions for a stationary tank
- a variation to, or a waiver of, secondary containment requirements
- an approval of a burner, a vaporiser or fuel dispenser
- an extension to a stationary container test certificate
- an extension to a location test certificate
- a waiver of pre-commissioning testing of a compressed gas cylinder
- an approval of a recognised inspection agency.
The application forms for all of the above approvals have been rebranded to WorkSafe forms, with updated contact details. Other than these changes, they are fundamentally the former EPA application forms. For a short period, WorkSafe will accept applications that are received on the old EPA forms (on the basis that they have already been filled in by an applicant but not yet sent to WorkSafe). If you have downloaded and saved to your computer hard drive or network the old EPA forms, please delete them and replace with the equivalent WorkSafe forms.
The registers for equipment (compressed gas cylinders, burners, dispensers and vaporisers, tanks and fabricators and tank wagons) and CSLs have also been rebranded to reflect the transfer to WorkSafe New Zealand by removing the EPA logo and replacing it with the WorkSafe logo.

Test certifiers must continue to enter test certificates into the test certificate register as normal. This process has not changed.
HSNO Codes of Practice and guidance information
HSNO codes of practice have not been rebranded into WorkSafe documents. This is because they were previously approved by the EPA and therefore have an official standing in law. Neither has the guidance and information previously published by the EPA been rebranded. This information is available on the WorkSafe website, but as EPA publications. They are still applicable and relevant until such time as the new Health and Safety at Work Act and the accompanying regulations come into effect.

The 0800 number
The toll free 0800 376 234 number now comes directly to the WorkSafe offices, not to the EPA. It will continue to be answered by Antony. This number should be used for all enquiries related to workplace activities and should be your first point of call for all matters related to test certification.
